    I think it only really works if Corporate told the front line about it. I used a similar form for a different company that I got from the corporate website and faxed it in. When I called to confirm the fax was received and understood I found out they didn’t even know this form was from corporate, they thought I had thrown something together and they voiced the preference for a simple fax that listed each order separately. This form looks good but unless Chipotle has made sure their front line knows it and can use it, it will only frustrate customers and employees alike.
